Are the threaded ends of the output shaft the same thread pitch size? I am rebuilding a D18 transfer case and the threads on the main part of the shaft are 3/4 x 16 but the threads on the Output clutch shaft (the small part that fits into the bushing on the large part) has threads that are 3/4 x 20 it looks like.

Do I have some parts from a much earlier jeep or maybe a Dana 20?

The output shafts from a Dana 20 are completely different, won't interchange. I have found that there are two different thread pitches on the output shafts just like you found. Normally they are the same front and rear. I've found that it seems more to relate to the age of the transfer case. The earlier ones seem to be more common to have the finer thread pitch and later ones seem to have the coarser thread pitch. The finer ones are usually the ones with the hole drilled in it for a cotter pin to keep the nut from backing off and the others use a locking nut. Nickmil.

The front output shafts at least came with two different threads, the original one on Tonka was coarser than a replacement one I bought. Same splines for the yolk, just had to get the parts supplier to send me the proper nut. IIRC the parts suppier said the newer ones were the fine thread.
