Mine was shot, so I started looking around for a replacement. I was able to degrease mine enough to find out that it was made by "Triangle Mfg. Co., Oshkosh, WI". And, it just so happens that they're still in business, and still make the part. They have them listed on their website here:

    http://www.triangleoshkosh.com/eng/...-bearings/side-flange-mounted-bearings-2-bolt

You'll need the one with a 7/8" bore and 2.75" on-center bolt pattern. I'll call them tomorrow and see if they'll quote me a single unit. Otherwise, the dang things are fairly hard to come by, at least, ones that will fit the stock mount. It's easy to find a 7/8" bore flange mounted bearing, but finding one that also measures 2.75" between centers for the bolts is difficult (most seem to be larger). So far, without talking to Triangle yet, the only place I've found that carries something that will work is this place on Amazon for $60 each(no-go at Grainger, McMaster, Reid Supply, etc.):

At the Triangle link above, you can also have your choice between an oil cup style or grease zerk style (and they have PDF drawings of each). Once I find out anything from Triangle on pricing, I'll update this thread for future reference.

I forgot to ask, does anyone know the length of the two shafts needed for the PTO? I know it's 7/8" diameter, but that's about it. I also need to source a u-joint (complete) for the middle; near the flange bearing.

The length of the shafts depends on the winch you are using and the powertrain you have. I.e. what engine and trans you have. It's not listed in your post and people using remote devices like I phones can't see a signature if you have it listed there.

My bad; I'm running a F134/T90/D18 with a Koenig Model 51 PTO and a Koenig Model C-100 (underslung) winch. I can always measure everything once it's installed and find out the correct length, but if I had a good ballpark number, I could always purchase the shafts now and then trim them a bit when it's time to install.

I can probably get those measurements for you this weekend. I have that setup on the '57 restoration project.
