    Is the center electrode broken off?

    Bad plug
    old plug
    detonation
    loose fragment of carbon
    etc.

    The center diode is gone, It almost looks to have melted away...?

    The plug is pretty new, less than 5000 miles.

    Could a lean condition have caused this?

    Scary!! Wonder what the top of the piston looks like? I would run an ohm check on that plug wire as well. Looks like excessive heat/spark or possibly defective plug.
